Darkest Dungeon II is not the 2016 game with better art, and readers who expect the Hamlet will be lost inside ten minutes. Red Hook released it on Steam on 8 May 2023, and it throws out estate management, town building, hero recruitment queues and dungeon crawling in favour of a run-based road trip: four heroes, one stagecoach, one road to the Mountain, and either you finish the Confession or the run ends. Progress between runs lives entirely in the Altar of Hope rather than in a town you rebuild.

Phase 1: Getting Started (The Valley)
The opening region of every run, used as a tutorial stretch. It is deliberately gentle and holds no lair boss, so treat it as the place to establish relationships rather than to hoard resources.
Level/Difficulty: Always region one of any run Key Rewards: Starting relics, early trinkets and a safe window to test a new party composition
What to Do in The Valley
- Learn The Stagecoach and road travel. The coach is your party's vehicle and your health bar for the road. Spend your first session getting comfortable with this.
- Pick Plague Doctor as your starting build. See our builds guide for why.
- Do not read first game guides for this one. There is no Hamlet, no estate, no roster of twenty five heroes, no stress relief buildings and no week counter. Everything you knew about provisioning and torch management applies to a different game.
- Acquire your first equipment upgrade: Blight damage over time or whatever's available.
- Clear all main content before moving on.

Phase 2: Early Game (The Foetor)
The rot and plague region. Its lair holds the Harvest Child, and enemy composition here punishes parties with no way to clear damage over time or heal between fights.
Level/Difficulty: Region two or three of a Confession run Key Rewards: Harvest Child lair trophy, Blight themed trinkets and relics
What to Do in The Foetor
- Work on Relationships between the four heroes. Heroes form positive or negative affinity with each other based on what happens in combat and at the Inn. This system becomes critical from here on.
- Farm for Blight damage over time if you haven't already. It's the key upgrade for this phase.
- Start with Denial. It runs two regions instead of three, which makes it the shortest complete loop and the fastest way to bank Candles of Hope for the Altar.
- Complete all objectives before pushing to The Tangle.
- Consider whether Man-at-Arms might suit your playstyle better than Plague Doctor.
Phase 2 Checklist
- Relationships between the four heroes integrated into gameplay
- Blight damage over time acquired
- The Foetor fully cleared
- Ready for The Tangle
Phase 3: Mid Game (The Tangle)
The overgrown, corrupted region whose lair belongs to the Dreaming General. Enemies here favour heavy single hits over swarms.
Level/Difficulty: Region two or three of a Confession run Key Rewards: Dreaming General lair trophy, plus the boss trophy required to reach the Mountain in Acts two onward
What to Do in The Tangle
- Master Combat tokens instead of flat percentages. Buffs and debuffs are visible tokens on portraits rather than hidden numbers. This unlocks a new layer of gameplay.
- Start working toward Bleed damage over time. It's the best equipment and becomes accessible around now.
- Treat relationships as a resource you spend deliberately. Positive affinity gives real combat bonuses, and a hero pair that has slid into hatred will refuse actions at the worst moment. The Inn is where you steer this.
- This area is the main skill check. If you can clear it, you're ready for late game.
- Start investing in The Flame in place of the torch for the tactical depth you'll need going forward.
Phase 3 Checklist

Phase 4: Late Game (The Sprawl)
The burnt industrial region, home to the Librarian. Fights here often mix ranged attackers with back rank support that must be reached.

What to Do in The Sprawl
- Finalize your build. You should be running Man-at-Arms or Plague Doctor with optimized gear.
- Bleed damage over time should be your primary. If you don't have it yet, prioritize getting it.
- Read the token board before acting, not after. Block, Dodge, Vulnerable and Weakness are visible stacks, so a turn that would have been wasted into a Block token is entirely avoidable information.
- Candles of Hope and the Altar of Hope optimization starts here. Small improvements compound into massive advantages.
- Farm this area for the resources needed to push into The Shroud.
Phase 4 Checklist

Phase 5: Endgame (The Shroud)
The drowned coastal region whose lair holds the Leviathan. Note that the Sluice, the other water themed region, has no lair and therefore no lair boss at all.
Level/Difficulty: Region two or three of a Confession run Key Rewards: Leviathan lair trophy and Shroud specific trinkets
What to Do in The Shroud
- The Shroud tests everything. Come prepared with your best build and gear.
- Stress caps at ten, and a hero at ten risks a Meltdown that resets their relationships and leaves them on one health point. Spend Inn items on stress before it becomes a crisis rather than after.
- The endgame loop: run The Shroud, optimize gear, push harder content.
- Experiment with Vestal for a fresh take once you've mastered the standard builds.

- Approaching it like the 2016 original and hunting for a town to upgrade between runs.
