Ndemic Creations brought Plague Inc: Evolved out of Early Access on 18 February 2016, rebuilding the 2012 mobile game with a larger world map, custom scenarios and Steam Workshop support. You seed one of seven standard plague types in a single country and spend DNA points across Transmission, Symptoms and Abilities until every country on Earth is infected. The cure runs on its own clock funded by wealthy nations, so the entire game is a race between an infection curve you control and a research bar you can only slow.
Your choice of equipment defines your damage output and playstyle. This guide ranks every major option, explains what makes each one strong, and tells you which builds they pair best with.
Equipment Rankings
| Equipment | Tier | Best For | Notes |
|---|---|---|---|
| Air 2 | S | Reaching distant countries from a well connected starting nation | The second tier of airborne transmission, and the single most valuable early purchase for a plague starting somewhere with heavy air traffic. |
| Water 2 | A | Infecting islands before ports close | The naval counterpart to Air 2, and the reason island nations get infected before they close their ports. |
| Cold Resistance 2 | A | Greenland, Iceland, Canada and northern Russia | Cold climates block infection outright without it, which is what strands runs at Greenland, Iceland and northern Russia. |
| Genetic Hardening 2 | B | Brutal and Mega Brutal runs where the cure finishes first | Raises the amount of research needed to complete the cure, which is the only lever that directly attacks the losing condition. |
| Total Organ Failure | B | The final lethality burst once every country is infected | The deadliest single symptom in the game at 25 lethality, well ahead of Hemorrhagic Shock at 15 and Necrosis at 13. |
S-Tier: Air 2
The second tier of airborne transmission, and the single most valuable early purchase for a plague starting somewhere with heavy air traffic. It is what makes Saudi Arabia's airport connections pay off.

| Tier | S |
| Best For | Reaching distant countries from a well connected starting nation |
| Key Mechanic | DNA points and bubble popping |
| Recommended Build | Bacteria |
How to get it: Available from mid-game onward. See our walkthrough for the exact progression point.
Best builds using Air 2: Pairs naturally with Bacteria thanks to how their stat scaling lines up.
A-Tier: Water 2
The naval counterpart to Air 2, and the reason island nations get infected before they close their ports. Greenland, Iceland, New Zealand, Madagascar and the Caribbean all shut down once the world panics, so Water 2 needs to land before that.

B-Tier: Total Organ Failure
The deadliest single symptom in the game at 25 lethality, well ahead of Hemorrhagic Shock at 15 and Necrosis at 13. It belongs at the very end of a run, evolved in the same burst as everything else lethal.

Equipment Progression Path
The order you should acquire equipment as you progress:
- Starting gear → Enough to clear Saudi Arabia
- Water 2 → Your first real upgrade, available early
- Cold Resistance 2 → Mid-game upgrade with good scaling
- Air 2 → Best-in-slot, aim for this by late game
- Upgrade to max → The power spike from max upgrades is enormous
Equipment Tips
- Always upgrade your primary equipment before diversifying. One strong option beats three mediocre ones.
- Match your equipment to your build's stat priorities for maximum scaling.
- The sequence that beats every difficulty is the same one: buy transmission and resistances until every country is infected, devolve stray symptoms as they appear, then evolve the entire lethality package in one burst. Losses on Brutal and above almost always trace back to evolving a cough too early.
- Check our tier list for how equipment rankings affect overall build effectiveness.
